How to convert a scanned PDF into a fillable form
Lots of businesses still have a form that only exists on paper: the original file is long gone, and every new copy is a photocopy of a photocopy. You can still turn it into a fillable PDF. A scanned PDF is really a picture of the page, so the job is to get a clean picture, then place form fields on top of it. The quality of the scan decides how good the finished form looks.
Step 1: get the best scan you can
- Start from a blank original. Use the cleanest unfilled copy you have. Never scan a copy someone has filled in; besides privacy, their handwriting ends up in every future form.
- Scan, do not photograph, if you can. A flatbed or sheet-fed scanner keeps the page straight. If a phone is all you have, use a scanning app that flattens and crops the page, in good, even light.
- Use 300 dpi in grayscale or black and white. Higher resolutions make large files without looking better on screen.
- Keep it straight. A page scanned at a slight angle makes every field look crooked. Most scanner software has a deskew or straighten option.
- One page per page. Do not scan two pages side by side onto one sheet.
Step 2 (optional): clean it up
If the page has speckles, shadows at the edges, or old pen marks, a free image editor such as GIMP can remove them before you build the form. Whiting out stray marks inside answer boxes helps typed answers stand out.
What about OCR (text recognition)? It is not required: form fields work on top of an image. OCR helps if you want the form’s text to be searchable or selectable, and some paid tools use it to detect fields automatically.
Step 3: add the fields for free in LibreOffice Draw
Because a scan is just an image, LibreOffice Draw handles it well. There is no font substitution to worry about.
- Open the scanned PDF with LibreOffice Draw (or insert the scan image onto a blank page with Insert → Image).
- Check the page size matches the original, for example US Letter.
- Turn on View → Toolbars → Form Controls and click Design Mode.
- Draw a Text Box over each blank line and a Check Box over each printed box. Zoom in so the edges line up.
- Right-click each field, open Control Properties, give it a unique name and, if you like, set Border to Without frame so only the scan’s own lines show.
- Export with File → Export As → Export as PDF, ticking Create PDF form.
- Test it in a browser and a PDF reader, typing long answers to check they fit.
The same steps work for any flat PDF; see adding fillable fields to a PDF for more on field types.
Paid option: auto-detection
Adobe Acrobat’s Prepare a form tool (a paid plan) can convert a scanned paper form and detect fields automatically. On a clean, straight scan this saves time. On a faint or crooked one it tends to miss boxes or place fields slightly off, so plan to review every field.
Should you retype the form instead?
Sometimes a scan is too poor to use: faded, skewed, or covered in marks. Then it can be quicker to retype the form in a word processor and make that fillable (see Word to fillable PDF). You get crisp text and a file you can edit next year. The trade-off is time, and you need to proofread the retyped wording carefully.
What we need from a scan
We take readable, blank scans. If the text is legible and the lines are visible, we can usually place fields over it. Send the clearest copy you have as a link from Google Drive, Dropbox or OneDrive. If we can’t make your form fillable from the scan you send, you can ask for a full refund, as our terms explain. We do not retype or redesign forms; we add fields to the page you send.
Price: $29 for 1–3 pages, $59 for 4–10 pages, with one round of fixes included. See converting a PDF to a fillable form for how the service works.
FAQ
Can a scanned PDF be made fillable?
Usually yes, if the scan is readable. Fields are placed on top of the scanned image, so the page looks the same and people type into the boxes.
Do I need OCR to make a scanned form fillable?
No. OCR makes the text searchable and helps some tools detect fields automatically, but fields can be added over an image without it.
What resolution should I scan at?
300 dpi in grayscale or black and white is a good balance between sharpness and file size.
Can I send you a scan that someone has already filled in?
No. We only accept blank templates. Files that contain anyone’s personal information are deleted unread, and we ask for a blank copy.
Rather not do it yourself?
Send a link to a clean, blank scan. $29 for 1–3 pages, $59 for 4–10, usually back within 24 hours.
